    Your NDIS Journey

    Reviewing Your NDIS Plan

Regular plan reviews ensure your supports continue to meet your needs and goals.

    When Do Reviews Happen?

Your NDIS plan is typically reviewed every 12 months. However, you can request an earlier review if your circumstances change significantly or your current supports are no longer meeting your needs.

Important: The NDIS will contact you before your plan end date to schedule a review meeting.

    Preparing for Your Review

Being well-prepared for your plan review helps ensure you get the supports you need. Here's what to gather:

    Progress on Goals

Document what goals you've achieved and what you're still working on

    Changes in Circumstances

Note any changes in your health, living situation, or support needs

    Service Provider Reports

Collect progress reports from your support providers

    Updated Medical Evidence

Gather recent reports from healthcare professionals

    During the Review Meeting

Your review meeting is an opportunity to discuss what's working and what needs to change. Be prepared to talk about:

    • •Your progress towards current goals
    • •New goals you want to set
    • •Changes in your support needs
    • •What's working well and what isn't
    • •Any new supports you'd like to access

Tip: You can bring a support person, advocate, or family member to your review meeting.

    After Your Review

After your review meeting, the NDIS will:

    1. 1.Assess the information discussed and any new evidence provided
    2. 2.Make decisions about your ongoing or new supports
    3. 3.Send you your new plan, usually within a few weeks
    4. 4.Schedule your next plan review date

    Requesting an Early Review

You can request an early plan review if you experience significant changes, such as:

    • •Your disability support needs have changed significantly
    • •You've achieved your goals earlier than expected
    • •Your current supports are no longer appropriate
    • •Major life changes (moving house, changing employment, etc.)
    • •Your health has deteriorated or improved substantially
